Online delivery is a fast and secure way for University of Iowa employees to receive year-end W-2 and Affordable Care Act (ACA) 1095 forms.

Employees who choose online delivery can access their tax forms in Employee Self-Service about a week before paper forms mail. They also avoid having their Social Security numbers sent through the mail.

To set your delivery preferences, log in to Employee Self-Service, go to the “Personal” tab, and click the “Form Delivery Options” link in the “General” section. You can choose online delivery for your W-2 forms and 1095 forms.

In past years, the UI has mailed 1095 forms (which serve as proof of health insurance) as required by the Affordable Care Act. This year, employees can choose online delivery.

If you have previously chosen online delivery for W-2 forms, that selection remains in effect—simply add your 1095 form selection to receive both forms online. Set your delivery preferences for 2017 forms by Jan. 9.

Once you choose online delivery, you will receive an email in mid-to-late January notifying you that your forms are available in Employee Self-Service.
