University issues important notices for the 2015 tax year
Thursday, December 17, 2015

Tax season is just around the corner, and the University of Iowa has issued two important notices regarding forms for the 2015 tax year.

New tax document regarding health insurance

If you had health insurance coverage through the University of Iowa or were eligible for health coverage during calendar year 2015, you will be receiving  a new tax form from the university for the 2015 tax year. This will be mailed to your home address by the end of January 2016. The university is also required to file a copy of this form with the IRS.

You will receive either a 1095-C form or a 1095-B form:

  • 1095-C form, Employer-Provided Health Insurance Offer and Coverage, will be sent to you if you were employed by the university during 2015
  • 1095-B form, Health Coverage, will be sent to you if had coverage as a retiree or a student and were not employed by the university during 2015

Do not discard this form. You will need it for your tax records when you file your 2015 federal tax return as proof of the health insurance coverage offered to you and your family (if applicable) during the year. Only one form will be sent to you. If you have dependents covered under your plan, you may need to provide a copy of the form to them for filing their tax return.

For more information, including an example of the 2015 1095-C or 1095-B form, see the University Benefits website.

If you do not receive either a 1095-C or 1095-B form by Feb. 1, 2016, please contact the University Benefits Office, benefits@uiowa.edu, 319-335-2676.

UI encourages electronic delivery of W-2 form

The 2015 W-2 forms are not currently on the Employee Self Service website, but if you want them as soon as they are available, sign up to receive your W-2 electronically.

You may request electronic delivery by checking the appropriate box within the "Stop Paper Mailings" option located in the Personal tab of Employee Self Service.

The electronic version will serve as your official document and is available on a secure site. By selecting the electronic version, your Social Security number will not be sent through the U.S. Postal Service and your forms will be available approximately one week before the paper forms will be mailed.

Once you have received notice that the electronic version is available, you can view and print your 2015 W-2 from any computer that is connected to the campus network or from an off-campus computer if you have enrolled in the University of Iowa Duo Security Two-Step Verification program.

You may consent or revoke your election to receive your W-2 electronically until Jan. 9, 2016. If you consented to receive your form electronically for the prior year, your consent will remain unless you change it. If you do not consent to receive your W-2 electronically, the University of Iowa will send your form through the U.S. Postal Service.

For additional information and software requirements, please see the View Year-End Tax Information section under the Personal tab of the Self Service site.
