Mercy Family Pharmacy in Dubuque provides quality, cost effective patient care

The University of Iowa College of Pharmacy’s residency site in Dubuque was recently recognized as “a model of excellence” by the American Pharmacist Association (APhA).

UI’s College of Pharmacy has partnered with Mercy Family Pharmacy in Dubuque as a PGY1 Community Pharmacy Residency site. Mercy Family Pharmacy has become the in-house pharmacy for the Crescent Community Health Center (CCHC). The primary goal of CCHC and Mercy Family Pharmacy is to provide high-quality and cost-effective care for all of its patients, including individuals on Medicaid and Medicare, as well as those who are uninsured and underinsured.

Mercy Family Pharmacy also provides and manages the patient assistance program for CCHC. Through collaboration with CCHC, Mercy Family Pharmacy is able to offer a wide range of services to their patients including medication therapy management services and medication counseling and education.

In addition to the programs mentioned above, CCHC and Mercy Family Pharmacy have launched the following initiatives:

  • Integrated care model with primary care, clinical, and dispensing pharmacy services
  • Postgraduate year 1 Community Pharmacy Resident program with a focus on clinical pharmacy practice–based research
  • Physician–pharmacy collaborative practice agreements for specialty clinics in anticoagulation, diabetes, hypertension, and asthma
  • Strong organizational support and a developed business model that provides resources for clinical and dispensing pharmacy services
  • Integrating revenue maximization, perpetual inventory, auditing, and reporting systems into their operating procedures
  • Participation in the HRSA 340B Peer-to-Peer Network as a recognized Leading Practice Site
